Request for Reasonable Accommodation
At Rocky Mountain Youth Corps (RMYC), we are committed to creating an accessible, supportive workplace where all employees can succeed. This Request for Reasonable Accommodation process is designed to support employees who may have a physical or mental condition that impacts their work. Through an interactive and respectful process, we aim to identify reasonable accommodations that enable employees to perform the essential functions of their role.
How to Use This Form
If you are requesting a workplace accommodation, you may be asked to have your licensed health care provider complete the Medical Provider Certification Form. This form helps RMYC better understand any functional limitations related to your job and identify appropriate accommodations.
